Semaglutide: The Compound the Other Four Are Implicitly Measured Against
The only peptide on this desk with a genuinely large, repeated, positive outcome-trial record across multiple populations — and even here, the evidence has real limits worth stating plainly.
The short version
Semaglutide is a lab-made version of a natural gut hormone, GLP-1, engineered to last about a week in the body instead of a couple of minutes. It is an FDA-approved prescription medicine, not a research chemical, used for type 2 diabetes, chronic weight management, reducing cardiovascular risk in people who already have heart disease, and — since 2025 — a form of fatty liver disease. It comes as a weekly injection or a daily pill.
Of the five compounds on this desk, semaglutide has by a wide margin the largest and most rigorous evidence base: multiple randomized trials, some enrolling tens of thousands of participants, run specifically to test hard outcomes like heart attacks and kidney failure, not just weight on a scale. That is precisely why it is treated as the benchmark here rather than because it is somehow the most interesting compound scientifically. This page states its real trial results, and it also states plainly where semaglutide lost a head-to-head comparison and where its evidence has genuine gaps. It recommends no dose for any individual.
What it is
Semaglutide is a 31-amino-acid analog of human glucagon-like peptide-1 (GLP-1), sharing roughly 94% of its sequence with the natural hormone. Two changes give it its defining pharmacokinetic property: a substitution at position 8 blocks the enzyme (DPP-4) that normally destroys native GLP-1 within about two minutes, and a fatty-acid side chain attached at position 26 binds strongly and reversibly to serum albumin in the blood. That albumin binding shields the peptide from clearance and breakdown, producing a roughly week-long circulating presence that is the entire structural basis for once-weekly dosing.
The oral tablet formulation is a separate engineering problem: it is co-formulated with an absorption enhancer that briefly raises stomach-surface pH long enough for a small fraction of the dose to be absorbed before acid degrades it. Oral bioavailability is low — under 1% of an oral tablet dose typically reaches circulation — which is why the oral form requires strict fasted dosing with minimal water and no other food or medication nearby.
How it works
Semaglutide activates GLP-1 receptors throughout the body. In the pancreas, it boosts insulin release only when blood sugar is already elevated (glucose-dependent), which limits the risk of dangerously low blood sugar when used alone, and it suppresses inappropriate glucagon release from the pancreas's alpha cells. It also slows gastric emptying, which blunts the post-meal blood-sugar spike and is part of why nausea is such a common early side effect.
The weight-loss effect is understood to be primarily a brain effect rather than a gut effect: semaglutide reaches appetite-regulating circuits in the hypothalamus and brainstem, activating neurons that signal fullness and suppressing neurons that drive hunger, and it does this without measurably lowering the body's baseline energy expenditure — the weight comes off because less is eaten, not because more is burned [26]. Beyond glucose and appetite, GLP-1 receptors in the heart and kidneys appear to mediate separate protective effects that large outcome trials have now confirmed directly, rather than merely inferred from weight loss alone [22][23].
What the research shows
Head-to-head against a dual agonist — semaglutide's honest limitation. In a 72-week, open-label trial of 751 adults with obesity, tirzepatide, a dual GIP/GLP-1 receptor agonist, produced significantly greater weight loss than semaglutide — 20.2% versus 13.7%, a roughly 6.5-percentage-point gap [21]. This is worth stating first, not last: semaglutide is the benchmark on this desk for its overall evidence base, not because it wins every comparison.
Weight management. In 1,961 adults with overweight or obesity and no diabetes, once-weekly semaglutide produced a mean 14.9% body-weight reduction at 68 weeks versus 2.4% with placebo [24] — the trial that established the weight-management indication.
Cardiovascular outcomes. In over 17,600 adults with existing cardiovascular disease and overweight or obesity but no diabetes, semaglutide reduced the combined rate of cardiovascular death, heart attack, or stroke by 20% relative to placebo [23] — a genuine hard-outcome trial, not a surrogate weight or glucose endpoint.
Kidney outcomes. In 3,533 adults with type 2 diabetes and chronic kidney disease, semaglutide reduced the risk of major kidney-disease events by 24% versus placebo [22] — another hard-outcome trial in a population selected specifically to test it.
Earlier cardiovascular trial, with a caveat. In an earlier trial of 3,297 adults with type 2 diabetes at cardiovascular risk, semaglutide reduced cardiovascular events, but retinopathy complications occurred significantly more often in the semaglutide group — a signal thought to reflect harm from rapid blood-sugar correction in people who already had retinopathy, rather than a direct toxic effect [27].
Mechanism, not outcome. Rodent studies mapped exactly where in the brain semaglutide acts to reduce food intake, without reducing energy expenditure [26] — useful mechanistic grounding, but an animal study, included here because it explains the human trial results above rather than adding a new efficacy claim.
Dedicated safety review. An independent safety review concludes semaglutide's overall risk-benefit balance is favorable in type 2 diabetes, with gastrointestinal effects as the dominant, mostly transient adverse-effect category, alongside a real (if weight-loss-attributable) increase in gallbladder disease risk, and pancreatic- and thyroid-cancer signals it explicitly says are too rare to draw firm conclusions from either way [25].
Reported effects, cautions & safety
Community-reported experience with semaglutide is widespread, given how long it has been on the market, but it remains anecdotal, not clinical evidence — self-reported, unverified, and separate from the trial data above.
The most consistently described benefit is a quieting of persistent food-related thoughts within the first one to two weeks, alongside sharply reduced cravings for sugar and fried food. Most reviewers describe steady weight loss over months, and people using semaglutide for diabetes commonly report meaningfully improved blood-sugar readings. A widely discussed secondary effect is reduced interest in alcohol.
On the adverse side, nausea is the single most reported complaint, described by roughly a third of reviewers and typically worst in the first weeks and after dose increases [25]. Distinctive sulfur-smelling burps, alternating constipation and diarrhea, acid reflux, early fatigue on injection day, and occasional headaches are also frequently described, along with food aversions and, less often, hair shedding tied to rapid weight loss rather than to a direct drug effect.
The clinically documented cautions are more specific than the anecdotal ones. Gastrointestinal intolerance during dose escalation is the leading adverse effect and the primary reason people discontinue [25]. Semaglutide carries a boxed warning for thyroid C-cell tumors based on rodent data, making a personal or family history of medullary thyroid carcinoma or MEN-2 a contraindication, even though the human signal itself remains unconfirmed [25]. Acute pancreatitis is a class warning, again without a confirmed quantitative risk increase in humans [25]. Gallbladder and biliary disease risk is a real, if largely weight-loss-attributable, finding [25]. In people with pre-existing diabetic retinopathy, rapid blood-sugar correction on semaglutide was linked to significantly more retinopathy complications in a dedicated trial [27][25]. Body-composition substudies show a meaningful share of lost weight is lean muscle, raising a sarcopenia concern for older adults, and extension data show substantial weight regain after stopping, which is why it is framed as a chronic rather than curative therapy. The oral tablet's low bioavailability means it must be taken fasted, with minimal water nearby, or its effective dose drops considerably.
Where it fits in the evidence hierarchy
Semaglutide is the lead compound on this desk because its evidence base is categorically different from the other four: repeated, large, randomized, hard-outcome trials across multiple distinct populations and endpoints [21][22][23][24][27], not a single pharmacokinetic study or one trial that missed its mark. That said, being the best-evidenced compound here is not the same as being unbeatable or without limits — retatrutide already outperforms it on raw Phase 2 weight-loss numbers [19], and semaglutide lost its own head-to-head trial against tirzepatide [21]. Compared with CJC-1295, ipamorelin, and KPV — none of which has a completed human efficacy trial supporting its marketed uses — the gap is not close.
